Looks like a shady AirBnb where the host didn’t have a bathroom, but did have a spare basement storage space and a cow rug that was too ugly to put anywhere else and simply didn’t give a shit.
I wonder if that's the hot water heater for the whole house? Looks a bit small and it's odd to mount that on the wall unless modern stuff is just that good.
Something about this seems like someone's partially finished basement and it's a Pittsburgh potty, which answers some of your questions at least.
