Chronic Heart Failure. Doesn't matter how much plot armor you have, even Goku was powerless against bacon.

But they don't do cognitive assessments for heart failure.

They do for people his age. Decreased blood flow to the brain and all. It's probably not just one thing. Like he also has a worn out rectum.
