Isn't that mostly in the line of intrusive thoughts, though? Like having a little voice in your head that goes "damn girl u hot today" sounds nice, but when you have impulses like, "aww look, my dog is sleeping, I SHOULD FUCKING CHOKE HIM OUT AND DUMP HIM OUT THE WINDOW" that's... less welcome.
The voices depicted in stories are never pleasant and that's why people get distressed over them.
I've a friend who lives with hallucinations. It's not so much that they struggle telling what's a hallucination and what's reality apart, it's more that they get in the way. They know that the forest that's replaced their hallway and has panicked animals running through it isn't real, but that doesn't make it less stressful.
