You are indeed missing the point, but yes that is the goal. You see, people often live outside of their means; and particularly April being expensive with the stupid war and that is what this photo represents with the captions contexts.
I think they mean that the boobs are the expenses, and the shirt is the salary. The salary is too small to cover the expenses.
