My nephew has been on T for a while now. He told his mom the deep voice was from COVID 🤣 he's planning on waiting till after top surgery to tell her so she doesn't kick him off her insurance. I think she'll be supportive, but there's trust issues between them.
If there's even an inkling of doubt, it feels totally reasonable to hide treatment. I hope that they will understand, when your nephew finally comes out, just how important this is.
