Yeah, you don't need the urinal to be under the sink. You can just run a pipe to the urinal...

I've actually seen those (or rather some quite similar to them) in the wild a few times. They're not very convenient to use (because the seat is in the way) even though you can't help but think they're a great idea each time you use them.

I'm not going to bother to research, but I'm sure there must be models where the sink faces the side of the toilet. It'd take up more space, but still less space than a totally separate sink and toilet.

You have to straddle the toilet backwards to use it pressing your member against the back of the toilet and sit down after peeing. 4/5 Stars.

My favorites have the sink next to the toilet rather than in one piece, yes.
