Okay, so he's an ophthalmologist, and he's talking about what her potential prescription for her glasses might be.

It is easily misread as a misogynist measurement of her beauty, and he recognizes that and acknowledges it. It wasn't what he meant. Hence, the meme.

I'd never heard of this guy either. But I read all the way to the bottom and deduced from the last comment that his previous comment meant something other than the woman's appearance. It requires using intelligence.
